Session Description: Transitioning care from a pediatric to adult provider can be a stressful experience for patients and their families and poses a unique set of challenges for the adult endocrinologist. An awareness of relevant patient specific factors and development of a team-based approach to receiving these new patients can ease the transition for all involved. This session will provide clinicians with a comprehensive framework for approaching these transitions.
