In Journal of the Endocrine Society, Cardenas and colleagues review data on the perioperative management of patients on glucocorticoids and suggest treatment strategies. Findling and Raff, in an Expert Endocrine Consult, discuss two patients who were referred for inferior petrosal sinus sampling for the differential diagnosis of ACTH-dependent hypercortisolism and were discovered to have hypercortisolism from a covert alcohol use disorder and from chronic kidney disease. Warde et al. describe age-related changes in the adrenal glands, which lead to numerous problems, and assess possible strategies to mitigate these.

In JCEM Case Reports, Subbu and coauthors describe a patient who developed extensive osteonecrosis after prednisone therapy and achieved good resolution of pain after one dose of zolendronic acid. Kurematsu and associates describe three cases of patients with adrenal insufficiency having an adrenal crisis after COVID-19 vaccination. Park and colleagues describe the evaluation of an unusual case of pseudo-Cushing syndrome.

In Endocrine Reviews, Bertherat and coauthors discuss diagnostic and therapeutic progress in primary bilateral macronodular adrenal hyperplasia. Pofi and coauthors parse the manifold side-effects of treatment with glucocorticoids — prescribed for 1–3 % of the world’s population — and ask: can we separate the good from the bad? Langouche and associates cast a critical eye on the hypothalamus-pituitary-adrenocortical response to critical illness, questioning broad implementation of stress dose hydrocortisone treatment of patients suffering from acute septic shock on the assumption of cortisol insufficiency.

In Endocrinology, Vonderoh and associates report that piglets born vaginally have 30-fold higher levels of plasma FGF19 then piglets born through cesarean section, suggesting a perinatal role for maternal or exogenous glucocorticoids in the development of the bile acid-FGF19 pathway. Michael and coauthors review commonalities of adrenal steroid physiology involved in the regulation of both prostate and breast cancer. Le Billan et al. discuss the under-recognized role of steroid hormones (including glucocorticoids) in alternative splicing of pre-messenger RNA.

In JCEM, Sconfienza and colleagues provide a systematic review and meta-analysis of the prevalence of functioning adrenal incidentalomas. Kjellbom and others report that in a matched cohort study they found that non-functional adrenal adenomas posed no increased mortality risk in patients with adrenal incidentaloma, unlike mild autonomous cortisol secretion. Ramirez Alcantara and associates report that newborn screening for adrenoleukodystrophy leads to earlier detection of adrenal insufficiency and the potential for earlier initiation of effective treatment.
